There has been the occasional such incident every now and then ever
since the first motorways were opened.

I've seen it myself when someone attemped to enter the Dartford Tunnel many
years ago, and a Commonwealth cycling team tried to use Manchester's
Motorways 4 years ago.... but most of these tend to be hard shoulder
'users', not lane 3/4!
