Thanks for the general conclusion that a new line from BT might be the

Probably should have just left the line with Orange. What caused you to
want to revert to BT?
How long a list do you want?
The simplest is of course that on BT you can prefix calls where you like,
even (rather messily for CPS), but not if you have LLU'ed.
