If you know or can find out where the base station is then a directional
antenna (Yagi, like TV) pointing the right way would definitely help.

If the house has upper floors, so will going upstairs.

I've found an Ericsson R320s at the back of a drawer which seems to be the only handset in my possession with an external antenna connection (though a V220 and A1018s have screw-in stubs) but I don't see that connectors are readily available so that may be a non-starter.

I know exactly where the base station is located but, although standing at an upstairs window on the appropriate side of his house does help a bit (the walls are massively thick stone), this is far from convenient and the improvement isn't great.

Looks like he'll have to let calls go unanswered and go upstairs from time to time to collect messages or identify missed calls so he can return calls by landline instead of using included minutes. Perhaps QxTelecom would be of use in this latter situation.
