Lifestyle factors are the main determinants of chronic back pain in the
absence of defined problems (like slipped discs). Prime examples are
excess body weight and very low activity levels. Certainly in my own
experience my back gives me problems only when I am not active enough
and becoming more active has always fixed any problems. I have had
problems in the past with both my upper and lower back. I have been
incapacitated by both. When it comes to non specific chronic back pain
it seems safe to ignore the usual advice and use painkillers to allow
activity. I have certainly used activity as soon as acute attacks have
subsided enough to barely allow it. For eg I find pressups are useful in
controlling my upper back.
There is considerable evidence to back this up (pun intended) and I can
try and find references for you if you are interested.
