or that this is the policy of the RCC?

Both.

Of course, there will be oddities and hard cases (which notoriously
make bad laws), and the catholicity of a church implies that it is
covered by its relationship with the catholic church during (say) an
interregnum. But the fundamental principle remains - churches are led
by bishops.

What's the definition of "bishop", in this context?
