That was only the final error. As in so many incidents, there was a
lot more beforehand, including some by people far removed from the

Interesting how the two trains were routed to the same track.

It wasn't a failure of the interlocking though, the driver
missed a red signal.

There wasn't a failure of the equipment as installed. There was a failure to
install all the devices and systems that were supposed to have been
installed at the previous resignalling.
