I'd favour a requirement for an "urban driving permit" for HGV
drivers, which required some level of (regularly) demonstrated
competence on all smaller and more vulnerable classes of vehicle in
the urban environment.
Any built up area could then have "permit zones" where only drivers
holding such a permit were allowed.
Non permit drivers could then be restricted to motorways & trunk
routes.

I'd also favour a requirement for an "driving permit" for cyclists which
required some level of (regularly) demonstrated competence

Would you like to explain the costs of such a scheme,

What would the costs of your scheme be?

Perhaps the cyclist would bear the costs, after all you would probably expect the HGV drivers to bear their costs.

how it is policed,

How would your scheme be policed? It would have to entail some new scheme for licence display on the vehicle, wouldn't it?

License, cycle, do those words go togather?

the potential benefits

It would obviate conflict between cyclists and HGVs.

That's what you want, isn't it?

Are you sure he wants that?

versus the cost, and why no such
proposal has ever been seriously considered?

Probably for similar reasons as to why the HGV "urban permit" has never been seriously considered.

Indeed
