he gets out of his red van(2), and threatens to:

"You do that again and I'll take that fucking bike off you
and kick your (1) head in."

(1) I don't recall if there was another "fucking" here or not.

(2) two lanes of traffic plus his van between us.

This is where a helmet video camera is great - if they know you're
pointing a camera at them, it avoids this sort of threatening
behaviour in nearly all cases. Rather than gesturing at him I would
have pulled the camera off the handlebars and visibly pointed it at
his face and then at his registration plate.
