If you get rid of cars from the town centres you just drive (no pun
intended) everyone out to "out of town" shopping places - the sort which
are a pain in the arse to get to by anything other than a car.

Only if you assume that everyone has a car or wants to use it to go
shopping.
I think that town centres you could walk around in safely, with decent
public transport to get you there, would be a very attractive prospect
for a lot of people.
For those who prefer visiting a giant car park so that they can get
six months worth of shopping every week, the restrictions of a busy
town centre would have driven them out of town anyway.
