Dont want to abondon friend but difficult to cope

How do you deal with a friend who misinterprets what you say and do?

Its starting to get me down a lot I really care for her and dont want her to feel even more isolated but I feel like I am on eggshells now.
I will say something which is taken the wrong way she gets upset & unhappy with me.

It seems she is getting more irrational and even trying to talk about things causes more problems as she thinks I say things to hurt her even though I dont mean it. Everything seems to be reinforcing her negatives and she cannot she other explanations.

I care about her too much to walk away but find it hard to cope with her perceptions of reality.
