& the actual answer to the Q in the Subject of your Post is:
"It depends on the Web Server & how it's been Set".
All Web Servers (regardless of what actual Web Server Software they
are Running) have various Options & Configurations that can be Set.
Either by the Admins of it, or (in some cases) by the Users.
The Options & Configuration Settings decide what (if any) FileNames
are used for the Default Index page.
So...
What Web Server are you talking about?
Most Web Servers will accept (as a minimum) any of:
index.htm
index.html
default.htm
default.html
& most will accept those Names in either Upper or Lower Case
(or a mixture)

& there's another Option which Sets whether a Directory Listing
is given when no FileName (just a Directory Name) is specified.
If you're getting a "403 Forbidden" Error, rather than a "404 Not
Found" Error, then there are several other things you can try,
to pin down exactly what & where the prob is.
Can you give us an example URL of a Directory on the Server that
you are talking about?
al i can tell you is that its my webspace on the Viginmedia isp site
this is the root of my webspace and gives the index.html that is in
the root of this area
http://www.cadskill.pwp.blueyonder.co.uk/
as well as this i have about 30 othe folders that are holding a
variety of my webpages named sisu1, ***,pacer etc and so i get to
these by adding this folder to the above like
http://www.cadskill.pwp.blueyonder.co.uk/pacer
it seems that if these folders contain an index.html file all is well
but if he file is an index.htm file you get the error 403 you are not
uthorised to view this page - this site is forbidden for you to see.

Are you absolutely sure that the page giving you problems does not
have mixed case in the index filename??
ah now they are as index.HTM, that is the ones that are with the htm
extensions
