I would prefer to have the navigation menu come after the content in
the HTML, however it is laid out on the page. That way, there is no
real need for a skip link, especially if you use a few <link rel=...>s
to link to the home page, next, prev, up, index, contents &c.
If, for one reason or another, you have the navigation before the
content, and it's a lot of navigation - I go by "if it fills more than
one screen on Lynx - then yes, I think there is a lot to be said for
having a "Skip to content" link.

_Best_ practice would probably say no, other than in certain
exceptional and specialised cases. But I think for _pretty good_
practice, you're fine if you warn people they open in external pages.
Some would disagree. I don't know how various mobile devices and
accessibility tools interpret target="_new" to know how harmful to
usability it is.
