GPs need discouraging from unnecessarily overloading hospitals - but
bonuses are not the right mechanism
It may well be that too many patients are being passed on to hospitals
by over-nervous or lazy GPs who ought to be able to treat them
themselves, so a mechanism to curb this would not be a bad idea in
itself.
But any such system should be based solely on clinical judgment, and
not influenced by the prospect of extra cash.
Oxfordshire Primary Care Trust is so far the only part of the NHS that
is offering bonuses to doctors who send fewer patients on to
specialists.
But others could easily follow its example, taking advantage of new
Government rules.
If this happens, many people will be unsure whether they are getting
the proper treatment, and afraid that they have been denied it because
their GP is sharing in a handsome bonus scheme.
In a universal system such as the NHS there is always bound to be some
sort of rationing. Medical care can never be limitless or it would
eventually swallow the entire Gross National Product.
But, as the experience of NICE repeatedly shows, the public will not
tolerate decisions taken purely or mainly on grounds of price.
Oxfordshire PCT should immediately reconsider its bonus scheme, and
other trusts should make it clear that they will not follow this bad
example.
