Does having little names over everyone else's head really add that much

that's like saying all you do on platform games is jump over things.
there's a heck of a lot more to large scale player vs player combat than
mashing a button. obviously your ignorance of MMOs is showing here.

Test Drive Unlimited let you play against hundreds of other people at
once. FIFA 10 lets you have massive leagues of 10v10 matches. They don't
require a sub.

and here too, the multiplayer side of MMOs are a lot more complicated
than that

So they need a few servers, but MMORPGs forgo things like half-decent
visuals, dialogue, scripts and story-writing, or anything like that.
Compared to a decent off-line game, the dev costs for all but the online
server stuff must be negligible.

And yet again here too in the case of Lord Of The Rings there's a wealth
of story that has been created for this game.

As for the costs, of course money will be going to server stuff, not to
mention customer/technical support but it's also paying for ongoing
development including new content, bug fixes, enhancements to abilities
and existing content. Again in the case of LOTRO new "books" are being
added every few months with a heck of lot of new content in them.

obviously you've never played a decent MMO

I've played WoW. It involved a man saying "go there, press 1 a lot, come
back", over and over again.

That's how it starts, but then you can break down the mechanics of any
game to "press a button every so often". Once you get past the
initial areas it gets much more complex and involving, however you
won't get past the initial areas if you only play for a month.

And do you really think paying a monthly sub to go to "ongoing bug
fixes" is acceptable?

You amuse :) Out of the list mentioned, bug fixes is obviously the
only thing anyone is paying for, you are completely correct :D

Be honest, the reason you don't like MMO's is that you can't max out
your 1000 achievement points and move onto the next game ;)
