As has been mentioned, they don't do anything in the hand. When you turn the meta-game you can 'turn on' the skulls, which give you point multipliers in exchange for making the game harder (ranking up opponents, making their weapons only leave half the ammo, etc). When you find a skull in single-player you just pick it up, wait for the achievement to unlock and put it down again. There is one gold skull on each level, and there are four silver skulls throughout the game which don't carry achievements and don't offer multipliers, just have strange effects (like making the grunts explode into confetti when killed). Do not try and find the IWHBYD skull without using a walkthrough, cos you won't.
