actually include the date as well. You would do that in System
Preferences > International.

Yes I think that's how I did it last time, but I can't seem to do it now.
Mind you I hadn't realised that if I clicked on the time it showed me the
date underneath, I'm sure it didn't before :-) I'm happy enough with it
doing that.

I just managed this under 10.5.6. In "International", customise the date under the Format tab for say Medium, select all the formatting items, and then Edit->Copy.
Still under the Format tab, Customise the Medium date format and paste the copied items where you want them to appear, and add some spacing if you wish.
Doing this appears to turn off the seconds and the flashing time separators, so I had to go back into Date/Time to re-enable them.
As Simon says, it's a bodge and there is I imagine the danger that the date may now appear twice where some software is using the time/date formats to display a time and date.
I tried this and had a problem with dates in Excel.
