I set up a phpBB 2.0 server 4 years ago, and it's still running. It's
all open source. You need web server with PHP and a supported back-end
database such as MySQL. I've had no problems with security, but I did
apply every update when they were released, and I also didn't use any
third party MODs.

Back in August I upgraded to the latest Release Candidate of phpBB
3.0. It's vastly improved with more features, an unread thread option
that properly works, an more granular controls over permissions. It's
still at the RC stage, having reached RC7 a few weeks ago.

I've not seen any working threaded view in any PHP based boards I've
come across. One board called UBB did have such features, but the
software was exceedingly buggy with some glaring errors in the code
and quite awful support. I've no idea if it's still available.
