A generic Tiger install DVD
A week-old CCC clone of 10.4.9 on an external FW drive..?

One of two things. Either:
A) boot from the external FW drive (use Option-Reboot), format the drive
using Disk Utility, then use CCC to clone the drive back, reset the
Startup Disk in the Control Panel, and Robert's yer father's sibling.
Or
B) install 10.4 from fresh using the DVD, and then use Migration
Assistant to copy all your user stuff over.
A) will bring across everything ready to go, settings and prefs etc -
including anything iffy that might have been corrupted by the naff
drive.
B) will give you a nice clean start, but you may have to re-enter
passwords and registration codes, and probably re-install many apps
again (such as Photoshop).
You could even run off the External FW drive for a few days to see if
that setup is OK. If so, then just run the clone.

Yikes!
Went for option A) & got Name/Password dialog, but it's not accepting
my name or password...
Hmm, should, have worked, I've used the method enough times myself. You
can reset the master password by booting from the 10.4 installer DVD.
Check you haven't got the caps lock key on as well!
Oh, yes, also try installing AppleJack and let it run its set of tests:
