It's not supposed to; it doesn't re-add disks automatically. If it did you
could have the situation where a dying disk would be repeatedly removed and
re-added as it failed and then seemed to work. The only way the system will
automatically repair an array is if it had a spare disk standing by.

You need to tell the system to add the 'replacement' disk to the array with
the mdadm command; have a look at the manpage for the syntax details.

You shouldn't need to mess with the initrd at all unless there's something
else that's gone wrong.
