Like one of the other posts said its not straightforward.
There is a lot about making a bootable *DOS* CD here:http://www.nu2.nu/bootcd/
Needs to be this format to emulate the A: drive. It uses a loader, text
menu file and floppy disk images.
Rather than go on a steep learning curve and start from scratch, the
easiest way IMHO is to use an existing bootable CD, make an .iso image of
it and then edit the contents of the iso image.
If you want to give it a try I have upped an one of mine to rapidshare
(about 23 MB - a lot smaller than its windoze counterpart):

You will need some tools to do the editing, I use UltraISO to add / delete
files from the ISO and WinImage to edit the .img floppy disk files,
remember to leave any system files (command.com io.sys etc) alone. A
couple of the floppy images are 2.88 MB so you can get more on than a
regular 1.44 floppy. The text file 'diskemu.cmd' is a text batch file for
choosing what to use when the CD boots - straight forward to edit. The
diskimage(s) will appear as drive A: whatever else you put on the CD
appears on the CD drive letter.

weird.. I see no format.com or other standard dos commands one takes
for granted !!
no command.com and io.sys either. no autoexec.bat and config.sys.
What was this dos boot cd based on?
I can see it is more for ghost and restoring images.
googling a file deload.bin , I see a similar thought process here ,
and they are talking about one of Bart's things

I can see this is not a bat file. Bat files use choice.com and " if
errorlevel "(see DOS HELP.COM [1]). And not "onkey"..
Obviously some kind of dos clone .. Though the time I used a dos clone
(perhaps from one method of making powerquest partition magic boot
disks), it had files similar to command.com and io.sys, even similar
names. This one doesn't
I always heard that Ghost was very complicated, and I ignored it .
Until I tried Norton Ghost v8, which had a GUI and was jokingly easy.
But everybody says Acronis is even better than NG.
I looked because I have to make one of these CDs now! My one has
gone bad. I was gonna use yours! Doesn't matter, not a problem to
make.. I just used nero v6 last time, v easy. And avoided Bart's
instructions (though they are interesting, quite technical).
