I doubt they loosen significantly over time. The main problem is that
the pushpins have a tendency to break when you pull them out of the
board, so refreshing your thermal compound becomes a little risky.
They do seem to be a 'single installation' type design, fortunately, some
third party solutions exist which solve the problem.

This one solves the problem by replacing the backplate underneath the
motherboard, allowing you to screw in tight the heatsink and fan.
This does require a nomination for most stupid stock socket cooling solution
devised in recent times, with a special runner up award for the original
socket A design heatsink.
As the socket A became more widely used, heatsink/fan makers improved the
clip design, but even then, it was truly awful, often requring the removal
of the power supply due to the positioning of the socket clip. Some of the
metal clips where so stiff, that the amount of force required , was to be
frank, a bit silly, no wonder so many athlon cores got crunched...
