Actually is is called begging in the real world. Otherwise, you
can pay cash or do without. Libertarians posting here have long
argued that if we had universal coverage in the USA, then there would
not be "enough" medical care for the middle class.

I very deliberately didn't want to get into a discussion on social medicine.
You didn't address my questions about what would they do with the homeless
quadriplegic. I was trying to ask a serious question. Would they treat
him, and to what extent, and how long would they try to keep him alive,
