I can give an example from a primary source. Joseph Plumb Martin, in
his autobiography "The Adventures, etc, of Private Yankee Doodle",
describes getting up one morning at, IIRC, the Siege of Yorktown. He
says, "When I had got myself regulated, I went in search of food...."
He's using the term "regulated" in the exact same context as a modern
soldier would say, "Once I got my $#!+ together, I went looking for
breakfast".
"Well regulated" did not mean "controlled by rules" in that context.
"Getting your *** together" still means getting organised to me.
Then there are government regulations, laws and statutes for exactly how
you're going to get your *** together?
....or is this something that can be done without governmental
micromanagement?
*chuckle* yeah I can manage without directions most mornings :)
But if I want to hold some jobs I have to prove I have my *** together, as
in random drug testing, or as in eye testing to renew a drivers license.
If any standard it is set, the implication is that enforcement of some type
is needed.
In the case of the militia, how are we to know it is well regulated
(working properly, if you prefer) without anyone watching over it?
Simple, we do. It's called "Congress".

How can it be decided if it is well regulated without some sort of

Because Congress says it is, otherwise they would impose a higher level of
standards.
Do you know how are Congress monitoring or enforcing these standards to
know?
Would the founding fathers bothered to specify it be well regulated if
they didn't intend those words to mean something more than simply "a
militia"?
Nope.
What I fail to see however, is how regulating the militia in any manner
means you can regulate the people or their arms.
