Issue and Use of Ma Deuce

In WWII US infantry formations what was the scale of issue
of the M2 .50 HMG? Company weapons platoon or some higher
formation?
What was doctrine regarding employment.
It was a fairly unique weapon in infantry usage and available
in far larger numbers than equivalent items on the Axis side.
