Translation of Polish Birth Record

I have posted the birth record of Nachman Piekier on Viewmate. It can be viewed at

http://www.jewishgen.org/viewmate/viewmateview.asp?key=19928. My mother was one of seven siblings and this record of her brother's birth is the only record I have found for any of them.

There seems to be a lot of text with only a few handwritten entries. I would be grateful if someone could tell me what is contained in the text (either in summary or translation), what the handwritten entries say, and all genealogically important information that may be contained in this document.
