The Canadian Archives actually has their Attestation forms online at

Hope that comes out OK otherwise I'll have to tinyurl it.

I find these forms very interesting as quite a lot of these young men still
had family back in Britain and the N-O-K is often a Mother, Father or
sibling at an address this side of the pond. A description of the person is
given, details of any previous service and also whether they consented to
being vaccinated or not (an early CYA clause?)
