Another reason could be a decision to change the name of the child (or
middle name) - my friend was surprised to find what she thought were two
registrations in the same quarter for the same (unusual last name) with same
mother's maiden name. As far as they knew the child was a singleton, not a
twin - but ordered the cert only to find it was the same child whose name
had been switched after the initial registration.

Another reason for a re-registration is if there was a mistake of some sort
on the original registration, although there is usually some indication in
the index.
