AIUI, the name simply refers to the technique of adding an extra virgin to the olive tramplers, in order to squeeze more oil out of the fruit.

The best olive oil comes from countries where virginity is still prized.

If you buy the stuff which is "extra virgin" and "mechanically filtered", you'll notice the difference.
