Association of obesity and race with prostate cancer clinical
outcomes

Prostate cancer is a leading cause of death in elderly American men.
Obesity has been identified as a risk factor for poor prognosis in
prostate cancer patients. The goal of this study was to compare tumor
characteristics, diagnostic precursors and biochemical relapse free
survival (bRFS) in both obese and non-obese men by race. 824 prostate
cancer cases were identified at the University of Pennsylvania Health
System between 1995 and 2002. Obesity was measured using Body Mass
Index (BMI) from self-reported height and weight. Each participant's
BMI was categorized according to the WHO definitions of obese
(BMI>=30kg/m2), overweight (BMI 25-30kg/m2) and normal (BMI<=25 kg/
m2). For analysis, normal and overweight groups were combined (BMI<30
kg/m2) and compared to the obese group (BMI>= 30 kg/m2). Of the 824
patients, 233 (28.3%) were obese and 591 (71.7%) were not obese.
African Americans (AA) had higher BMI (P<0.0052) than European
Americans (EA). Race by itself was not a predictor of bRFS. Obese men
had higher tumor stage (Odds ratio (OR)=1.53 95% CI: 1.1-2.1) than non-
obese men. Obese AAs had higher tumor stages while EAs approached
significance (OR=2.4 95% CI: 1.08-5.32 and OR=1.40 95% CI: 0.98-2.0
respectively). Adjusting for tumor stage and age, we found obese AAs
significantly more likely to experience bRFS than non-obese AAs
(Hazard ratio (HR)= 3.35, 95%CI :1.12- 10.1), unlike their EA
counterparts (HR= .93, 95% CI :0.58- 1.47). Pooling all men and
adjusting for stage, age, and race, we found that obesity was not
associated with bRFS (HR= 1.13, 95% CI :0.75- 1.68). Obesity is
associated with higher stage disease for all men and poor prognosis
for AA men, suggesting for AA men, obesity may play a factor in poorer
clinical outcomes.
