Well, this is what happens when several failures compound to really screw
you up. I've often wondered if the long duration on orbit has some effect on
a spacecraft, given that its not very long after undock that a re entry is
attempted.
I do not know enough about the construction of the vehicle, but it seems to
me that the failure of separation in several recent vehicles with no
apparent cause needs to be looked at. Its quite obvious that this slow
shedding of a module is what caused the heating to the hatch area, and
indeed luck more than design seemed to sever the modules before a
significant amount of damage was done.
As for the wiring failure and smoke. This I suppose could be due to
overheating of a cable during the incorrect attitude re entry, but judging
from the comments about a previous wiring problem, I think its going to need
more investigation.
I'm surprised that if it was known what caused it last time, if they had not
taken stes to fix it on the one on orbit. However thy could not have fixed
the one just landed as it was on orbit at the time!
Seems to me that if we are going to go on long trips to other worlds, some
form of easy to fix construction will have to be used, as such things are
bound to be found on a mission way outside of any ability to fix it next
time!
