Well, for one, your PhD specialty doesn't have to be your IM specialty.
If you're a general internist, as oppose to a specialist, you should
have more flexibility during the final years of training for a research
fellowship/postdoc. I mean let's face it, the real reason for an MD,
aside from the clinical experience, is the knowledge that one can bring
home the bacon, seeing patients 3-4 days/wk, while pursuing one's
research interests w/o fear of never getting that uni full-time
position.

The first couple of years, however, you're a full time (actually more
than full time) worker bee under the guidance of both the physicians
and the senior residents. During this time, you'll have pretty much no
free time for anything other than food, rest, and a pickup game of
hoops to blow off some steam. Now, all residencies are a bit different
in the whole moonlighting/consulting work (vis-a-vis time off) for the
third and fourth year attendants. This is where you'll have to ask
around because there's a good chance that you can start your research
at this point in time but it's not a standardized option like let's say
fourth year of MD which is an open slate at many schools. Some programs
may never give you any leeway until it's all over.
