The tape came loose, eyelid opened and pulled loose end of tape across eye. Or, coming out of anesthesia you moved or put your hand up and touched it, unaware. Especially with eye ointment, oily or damp skin, the tape can loosen. Or, possible that the tip of ointment tube touched the cornea. With facial surgery, drapes, instruments or surgeon's hands could concieveably touch - if your eye came open under a drape, then they would be unaware. Hope you are ok :o)
If you are one of those patients who become very active going under or coming out, it is possible unanticipated movement caused something to touch your eye.
A report to Kaiser is unlikely to rise to the level of "not happen again". I would send a copy to head of anesthesia dept.
