Use trains if you plan to visit major cities. Buses are not an
alternative to rail, but a complement. They may connect cities with
other locations in the country. You will use public transport buses (or
trams or metro) inside the cities.

If instead you plan to tour mainly country locations, a car could be
better (buses may be unfrequent). Avoid however cars inside cities
(traffic, parking difficulty, restricted areas).

B&Bs are a relatively new thing in Italy. I've never used them myself
here, and the impression I got from guidebooks and advertising is that
they are not as cheaper w.r.t. hotels as in other countries. If you want
to go in the country you can consider "agriturismi" ... essentially
farms which also act as restaurants or B&B. The food will be genuine and
really good, not necessarily cheap.

Otherwise consider that hotels are rather expensive in major cities, be
them business or tourism goals. In such case they generally offer a b&b
fare, sometimes just a room fare. In places like sea or mountain
resorts instead the full board (pensione completa) or half board (mezza
pensione) arrangements are common, and convenient. They however require
a stay of at least 3 days.

Just to compare, for 50 euro you get just a lousy room with breakfast in
Rome, and you are lucky if you get a decent one for 70. For 70 per
person per day you can get decent full board in high season on lombard
mountains, for 45 per person per day you can get a gorgeous full board
(freshly baked cakes at breakfast, home made pasta and selection of meat
or fish at lunch and dinner plus enormous sweet buffet) in low season in
Romagna.

About itinerary, since this is a very long country, it really depends on
your preferences and interests. Art and history ? nature ? sea ?
mountains ?
