I've never waited on more than a 15-minute line in Italy, even in the
middle of summer. The trick is that most bus tours go sightseeing in
the morning and then give their passengers free time to go wandering
around in the afternoon. If you go to major museums at 2-3 PM, they
are much less crowded and the lines are minimal to non-existent. And
there is so much to see in Rome, Florence and Venice beyond just the
most famous sights and museums, and many lesser-known but interesting
places in those cities see very few tourists even in high season.

months in advance and buy tickets for a specific day? Is this true?"
Only if there is a special exhibition. That's not standard at all.
The only sight I know of that has that policy is the Alhambra in
Granada, Spain.
