Personally I would visit the places I would be considering before I
went. The Caribbean is a very big and widely varying place.

Things I need to plan:

Healthcare: Need to locate good healthcare facility in
case needed. We are both in good health but "things"
happen as you know

I don't think this will be a problem in Hawaii, but in the Caribbean
it will depend on where you are.

Transportation: Rent a car there? Or maybe buy two
mopeds and sell upon return? Maybe take bicycles?

In some places, you might want to buy a bike or car or moped there and
sell when you leave. Renting for the long term may be expensive.

Cell phone coverage: What is bets method for this? Take
TWO Sprint/Nextel cell phone walkie talkies? is
service and coverage decent on Big island?

This will depend on where you go. Probably best to buy a phone there
for the duration.

Living arrangements: Rent apartment for 2 months?
Condo? bed and breakfast? Don't want a lot of
amenities but do want shower and restroom and laundry
facilities. But I do like the Bohemian experience as
well...close to nature.

Renting an apartment would probably be the least expensive if you
could do it for the short term. It would depend on what season you
went. You don't say - if you go in the off season it will of course
be cheaper, but you may have hurricanes
..
Clothing" Shoes and rain gear and clothing? What is
needed and NOT needed?

Furniture: Any need to take camping chairs and tables
for rented condos?

Food: Want to cook as much as possible to save money.
Only eat out when the experience is wanted such as
great food. Or is it just cheaper and easier to eat out
daily?

We usually do our own breakfast and lunch and then eat dinner out
occasionally. Sometimes we have 'dinner' in the middle of the day as
lunch is cheaper and has plenty of food.

Inter-island travel: Might want to fly to other
islands especially Oahu to see historic stuff.

Well.... that's abt all I can think of for the moment

One thing you haven't mentioned is whether you can stay in a country
for that length of time without getting a special visa.

Islands in the Caribbean (or the Bahamas) tend to be expensive because
everything has to be trucked in. Those that are not expensive have a
very low standard of living and may also not have much in the way of
health care.

Given what you have said, I might recommend

Placencia in Belize - very laid back place and relatively cheap. You
could rent or buy a car there, and get to other places in Belize or
possibly in Guatemala English is the default language.

The Abacos or Exumas in the Bahamas - they use dollars and speak
English. Most of the islands you can ride bikes (although they drive
on the left) and you could take trips to other cayes. Or possibly
Bimini.

Playa del Carmen, Mexico. I have not been here although I have been
to Cozumel. They obviously speak Spanish but I think it is fairly
cheap

The Pacific Coast of Costa Rica - speak Spanish but is relatively
inexpensive and there are quite a bit of eco-tourism there.

Barbados in the shoulder season or a little earlier is not too
expensive - they speak English and drive on the left, but it is more
expensive and 'glossier'.
