Molesey and Kingston have head on collision in marshalling area.
Kingston sink and launch driver starts to reverse launch with motor
going towards people in water. Told to get lost in no uncertain terms
as drowning seems better option than decapitation by propellor.

Erm... I think never ever allowing your outboard to get anywhere near
people was about the third thing I ever learnt about driving a launch.
And the latter example from a safety boat?

Over the last couple of years, I've started to wonder if it should
become a requirement for any launch driver to by RYA lvl2 qualified
and for any boat that is involved in a safety plan to be driven by
someone with the RYA safety boat qualification. We don't use launches
all that much around here, but I've seen and heard of too many
dangerous drivers.
