Another case in point ... who actually pays attention and is helpful,
and are you paying for "doctor's" visits or "PA" visits with a
fraction of a doctor's time, if any.

In my business, systems administration, we see this a lot with our
vendors. It's a negotiation item because when we sign a contract for
x hours of an engineer, many times that translates to "field tech"
first, and then if escalation is required they bring in their
enginner. But they try to bill all of the hours at engineer rates. I
know this is pretty much across the board ... but it's still
frustrating as hell.
