Tom is correct if when he writes that with ONLY the hot water spigot being on you get cold water then it won't be the check valve.
There's hot water in the tank, right? If there's no hot water in the tank, fix the tank.
Assuming hot water in the tank, he learned this by cracking open the check valve, the mixing has to be AFTER the tank. Since the pipes don't meet except at the faucet, I'm not sure where the water would mix but there.

I think Steve is right. I have check valves on both sides of the hot water tank. Last year one went bad. It allowed an initial flow of hot water then restricted the flow. Just hot water barely trickled but anything less than straight hot was mostly cold.
