Well there's the (IMO) classic example of the tenor who was so miffed at his Carmen that he walked offstage in the last act, leaving her to die (or commit suicide?) without Don Jose's assistance. (It's supposedly a true tale, and the tenor involved reputed to be David Poleri, but I wasn't there, so can't vouch for its authenticity.)
