In all seriousness, no. If I get a patient of means coming in with a mouth like that, I know that there is a back story. It may be a pre-judgement, but that's the kind of thing all clinicians have to do.
It may be that he really doesn't care. It is more likely that he is afraid. He may in fact be ill, a current drug abuser, or any number of other things.
In any case, since we know it's not a money thing, I have to figure out exactly how this happened. And I can tell you, having seen this again and again and again over 30 years, it is extremely difficult to break through the problem and turn people like this around. You can only hope they get the help they need--and it's generally more help than I can provide alone.
