Yes, and you don't want to spend money needlessly (in fact, needless
registration has been one of the reasons the cost has gone up (among other
things) - they figure the more expensive they make it, the more serious
someone will be before they do it - but of course somewhere, there's some
kid blowing his weekly allowance on every single MIDI sequence he writes!)

One app per work, though there are specialized things for "collected works"
(see Matt's reply). But there are also differences between things that are
notated and that are recorded. You should really read the gov's circulars
and decide what's best for a given work. For me, I basically register
something if it's being performed and/or recorded (recorded so that it would
be played somewhere) outside of my control. I've had works performed on
local recitals where I don't register them and I collect the parts back from
the players. But if it's going out to people I don't know, that's when I
figure safer is better than sorry. Anything you post on the web, I would
register, unless it's something you don't care about (say an example).
