I've seen enough facts. They put a car on the segment in question and the control system didn't recognize its presence.

The facts are pretty cut and dried at this point. The control system malfunctioned and the driver tried to stop the train but action was taken too late to avoid a collision.

I also doubt that you're going to be able to fault the operator in any way. She was probably told in her training that the control system was completely reliable and she would never have to use the big red button. I'm also guessing that if one deploys the emergency brakes, injuries or perhaps even deaths will occur, and there would be a lot of questions to be answered. She really had to wait until she was SURE something was wrong. By that time it was a bit too late.

You find this kind of pressure in every transportation system. For example, an airline claims it puts safety first, but if a pilot makes the decision too many times not to take off or land in bad weather, they replace the pilot with one who will take a bit more risk.

What additional facts do you think could surface?
