Year ago in Montreal I had a molar that had abcessed and the rot had spread
into the bone. The dentist told me he couldn't use novocaine since there
was too much chance of spreading the infection into the bloodstream.

So he pulled the molar without novocaine - that was more than a tad
unpleasant.

But it was the next step that got me. He had to put the drill down into
the bloody socket from the removed tooth and drill away the infected bone.
I'd last about 30-45 seconds and then be on the verge of passing out, so
he'd take a break while I put my head between my knees and came around a
bit. Then he went right back at it again. I was up and down, up and down,
like a pump handle for 10 minutes.

Not something I'd care to repeat.

Then he was either a masochist or had concluded you deserved to be
tortured. There's no reason in the world that he could not have referred
you to a dental surgeon who would have used an anesthetist to put you
under while he did the work.
