You out there who say that *any* death is avoidable, how could I have
avoided this?
I don't think anyone _does_ say that, merely that the number of
unavoidable deaths is much lower than a naive assessment would suggest,
and is very nearly zero.
However if you were in the habit of immediately descending and ascending
stairs to generate the next DL with potentially lower difficulty monsters,
which is a good habit to have, I observe you would first have descended
without the pet. In this case it would not have helped but it is extremely
rare for this trap's boulder to be generated outside the room - indeed, I
would not have thought it was possible until now, although I can't find
anything in the source that forbids it. (Dylan?)
I think that is generally a good habit for survival so it is fair enough
to mention it - a few of the replies mention generally unwise approaches
which would happen to have helped in this specific case.
