Are they all identified? Ammunition which hits you is usually identified ("This must be an Orcish javelin +1!"), while the ammunition they were holding at time of death is not. Can't stack identified with non-identified items, even when common sense suggests that they are all the same thing. The other major cause of stacking woes is poison: if you do not have the option "smart ammo stacking" or whatever turned on, then each poison quantity is considered a unique item. So a sheaf arrow with 2 doses of poison cannot stack with a sheaf arrow with 3 doses of poison. Turn on smart stacking. It uses a law-of-averages type solution, which may give less precise results, but far FAR more usable.

 2. Time passes when I scroll the inventory menu? It's... rather
 harsh. I often find myself getting ambushed while I was fumbling
 with the controls (doh! Typed one too many down arrow). How about
 time passes only when I e[x]amine or move an item?

Time passes only when you take an inventory action. Scrolling will cause the "You are threatened" message to spam aggressively, but should not cause actual hits. If you're getting hit while just scrolling, this is a bug and ought to be reported. You can see the time costs of various actions in the help file.
