There is one situation where not having a ground wire is OK, and that's
if all the electric is run inside metal conduit. In that case, the
conduit acts as the ground return, so no third wire is necessary. (Also
makes a great nail guard! :) )
I'm pretty sure armored BX also falls into this category, but I'm sure
Bryan can confirm or repudiate that.
