Why? Someone screwed up. The 2 are keyed alike.
Probably the only cause, correct.
I'd install the diode. Hope ya ordered a couple caps and rectifiers, just in case :)

Newbie alert!
So I plug in the BSD, run through diagnostics to check things, and I
smell it.
Burning plastic/electronics/something.
I pull the plug and seconds later, smoke is billowing from the
backbox.
Open it up, and sure enough C11 is puffing away.
I've searched through threads here and see multiple possibilites. One
being J124 plugged into J128. Yup. That's how this one was set up.
More searching for a why this was this way and I cannot exactly figure
that out.
Anyone know why?
And is this most likely the only cause?
And lastly, should I install the diode as described elsewhere to
prevent excessive voltage to that cap?
I've ordered the replacement C11 and BR4 already, but I don't want to
replace them and find out it was something else.
Thanks in advance for any help on this.
RGP rocks when it comes to newbies and help. I can't tell you all how
much I appreciate your wealth of knowledge and experience with all
things pin and more.
