Nobody really tried all that hard. That's hardly the same as "nobody has
been able to." In fact, you just came up with a perfectly good in-
character explanation yourself:

If, for example, a character had to make some manner of psychic bond
to attune himself to an item in order to use the Daily (and higher
Tiers could attune more items) I could accept it, but that doesn't
match the rule as written.

Where's the problem? This is actually similar to how I thought of the
item limits working, inasmuch as I think about it at all. Also, the
heroes build up heroic momentum (for lack of a better term) as they face
foes, such that they can do more heroic things.

That's rather what I mean by plot abstraction - the only rationale
seems to be to encourage the BBEG trope. Personally, I suspect it's an
oversight. If they'd really meant people to be able to power up during
the course of an adventure, they'd have let anyone accumulate and use
multiple action points in an encounter. That's far more heroic than
using multiple item dailies.

What? It makes as much sense as spell preparation.

Spell preparation, when taken as 3e "preparation" rather than 1e
"memorisation" makes in-world sense. It's something of an a posteriori
justification for an existing rule, but it's not unreasonable.
