Ideally, start with a slightly isolated place, as your focus spot. Do
the broad strokes entire-planet first, as you've suggested yourself, but
when it becomes time to add detail, find some kind of island (or at
least a peninsula). It'll be somewhat isolated from the surroundings
(having just a few "points of contact), which has implications both for
culture and history, but also for making it naturally (slightly)
difficult for the PCs to just leave it.
In my one attempt at a homebrew, I set the adventure in a small duchy
that occupied the region around a river delta. It was a trading
nation, but had previously been the great power in the region. There
were two great nations, east and west, and no other foreign relations.
The ability to spend effort and time detailing one town can make that
town interesting enough to make up for its small size. Consider (in
FR terms) setting a campaign in Waterdeep - great big city, very
detailed, politics & intrigue - compared to setting one in the Silver
Marches - big, open terrain with long days travelling. A lot depends
on the flavour you want.
