You have not been trolled. A player honestly proposed this item to
me, and, judging purely by mechanics (forgetting about common sense or
dm fiat) it appears mechanically correct and legal.

Technically, there are no mechanics for creating item not included in
the DMG. There are just guidelines for designing, just like there are
guidelines for designing PrCs in the DMG, and common sense and DM fiat
is a necessary part of designing new game elements.

This is explicitly stated in the item creation rules:

"Not all items adhere to these formulas directly. The reasons for this
are several. First and foremost, these few formulas aren?t enough to
truly gauge the exact differences between items. The price of a magic
item may be modified based on its actual worth. The formulas only
provide a starting point. The pricing of scrolls assumes that, whenever

require at least some judgment calls."

Does that make it
not broken? Thats not really the question I suppose, but that is a
valid second question. First, I would like to know if anyone sees any
reason, per the rules, that this item would not cost this much.

Judgement call.

Secondarily, if there is common wisdom that says "Items that do X
(healing or whatever) cost differently than other items" then thats
fine, I just need to be able to quote something when I tell the player
no. As it is I have already allowed another player to create a
similar item, but it only activates on damage that occurred THAT
round, so you can't heal if you weren't injured that round. That way
you can't sit down after a battle and just heal all up instantly.

Assuming it's reasonably priced, that sounds just fine to me.

So, would you all deny this item, even if it is legal per the rules as
written? How would you require it be altered for you to approve it?

If it's supposed to be "healing on command, when I want it", a limit to
uses per day that's not on the order of thousands. :)

If it's supposed to be "constant healing at a reasonable rate", look
towards the ring of regeneration (which might well be overpriced, but
it's more reasonable than the shirt).
